Symposium Evaluation form

The information below provides technical specifications for the Industry Symposium Evaluation form and may also pertain to some EAACI Focused meetings with a digital component (sponsor item #VIR_SYMP2X.08X) that must be purchased if the sponsor would like to include/modify/customise their evaluation form from the standard questions detailed below.
Symposium Sponsors have the option to purchase an evaluation form that viewers/delegates are able to fill out for each Industry Symposium (see images below). There are three options available below, all Industry Symposia Sponsors must choose 1 of the following:
Option 1 – Use a standard industry evaluation form through the EAACI evaluation tool (see below example)
Option 2 – Choose your own customised questions using the EAACI evaluation tool (see below example) - Please provide feedback to congress@cyim.com until 1 week prior to the event (to be requested by the partners)
Option 3 – Choose your own customised evalution form/system (external link must be provided with other details to the official EAACI agency CYIM): Please provide feedback to congress@cyim.com until 1 week prior to the event (to be requested by the partners)

For each question, please tick the appropriate box, where 0 = Low and 10 = High

Option 2 Customised options (using the EAACI evaluation too) available
NB: Max. characters permitted including spaces are 100.
Wording of the 4 questions can be changed
1 to 4 questions can be deleted
1 or 2 questions can be added with rating or free text
Rating can be with figures (0 to 10) or a star icon
No colours or logos are included in the customisation
Option 3 Sponsor to provide own evaluation form/online platform (sponsor must provide a link to the external evaluation tool)
The data from the evaluation form will be made available by the end of the day of the Industry Symposium.
